Permission Slips
Lazy Mountain Bible Church has two different types of permission slips, depending on whether you're providing your own transportation or whether you're allowing the church to transport your child. Both types are available below,

• When you're providing your own transportation - "Get there yourself" permission slip for church events
• When church is providing transportation - "Car permission slip" required for church events

PARENTING TIPS:
