Permission Slips

Lazy Mountain Bible Church requires signed Permission Slips for your child to particpate in church-sponsored events. We have two different types of permission slips, depending on whether you're providing your own transportation or whether you're allowing the church to transport your child. Both types are available below.

Two types of Permission Slips - choose the one that's appropriate for the event:

When you're providing your own transportation - "Get there yourself" permission slip for church events

When church is providing transportation - "Car permission slip" required for church events
